Abstract Purpose:Among the various pathogens of infectious keratitis, Escherichia coliE. coli)is considered as a rare causative organism. We here report a case of infectious keratitis caused by E. coli, which had a background of chronic dacryocystitis.

Case:An 82-year-old woman with decreased vision, lid swelling and increased discharge in the left eye was referred to our hospital with a diagnosis of infectious keratitis. Initial treatment with topical moxifloxacin and cefmenoxime was unsuccessful. She was diagnosed with lacrimal duct obstruction a couple of years ago. On her initial visit, the best corrected visual acuity was left eye 0.01. An infectious ulcer with descemetocele was seen in the paracentral cornea. A culture of eye discharge showed a growth of E. coli, which was resistant to fluoroquinolones. After changing the prescription to topical arbekacin, lamellar keratoplasty was performed to prevent corneal perforation. Temporal settlement of infection was seen postoperatively, however, the melting of the corneal graft occurred after tapering of topical antibiotics. Silicone tube treatment for lacrimal obstruction was performed followed by secondary lamellar keratoplasty for corneal perforation.

Conclusion:E. coli has been recognized as a pathogen of dacryocystitis. Coexisting chronic dacryocystitis seemed to induce changes in the ocular surface microbiota, which resulted in the development of E. coli keratitis in this case.
