Abstract Purpose:To report a case of refractory intraocular inflammation post the administration of tumor necrosis factor-alpha inhibitor for scleritis associated with rheumatoid arthritis.

Case:A 69-year-old woman.

Findings and Clinical Course:Continued relapse of scleritis after application of systemic and local corticosteroid gradually resulted in scleral thinning. The additional administration of adalimumab helped control the scleritis;however, slit-lamp examination showed iritis, and polymerase chain reaction(PCR)of the aqueous humor revealed cytomegalovirus(CMV)associated iritis. Systemic valganciclovir and ganciclovir eye drops were successful in complete remission of the iritis, but diffuse vitreous opacity and hypopyon appeared after six months of discontinuation. PCR of the aqueous humor amplified the Epstein-Barr virus(EBV)-DNA, which indicated EBV associated uveitis. Iridocyclitis was temporarily relieved with the same anti-viral agents mentioned above;however, vitreous opacity worsened significantly. A biopsy of the vitreous body revealed high copies of EBV-DNA. Cytological examination from the vitreous specimen showed a small number of atypical lymphocytes. EBV associated lymphoproliferative disorder or lymphoma was suspected;however, the patient died of septic shock caused by systemic opportunistic infection.

Conclusion:While administering immunosuppressants, it is important to keep in mind not only systemic but also intraocular opportunistic infection and lymphoproliferative disorder.
