マイラゲル®が結膜穿破をきたした1例

Exposure of MIRAgel® in a case after surgery for retinal detachment

要約　目的：網膜剝離に対する強膜陥凹術で用いられたマイラゲル^®が上直筋を断裂して結膜を穿破し，眼表面に脱出した症例の報告。症例：76歳男性が右眼表面の異物で紹介され受診した。19年前に右眼に網膜剝離手術を受け，その直後から上転障害があった。2年前に右上眼瞼に腫脹があり，マイラゲル^®が移動してきたことが原因と考えられた。所見：矯正視力は右0.1，左1.2で，右眼は下転していた。瞼裂に亀裂し石灰化したマイラゲル^®が露出していた。CT画像での直径は10mmであった。マイラゲル^®の除去手術中の所見として，上直筋は確認できなかった。手術のあと右眼の第1眼位は改善し，複視は減少した。結論：マイラゲル^®などが眼外に脱出した際には，その早期除去が望ましい。

Abstract.　Purpose：To a case of exposed buckling material on the ocular surface. Case：A 76-year-old male was referred to us for exposure of buckling material through the conjunctiva in the right eye. He had received surgery for retinal detachment in the right eye 19 years before. He had had impaired upward gaze soon after surgery. He showed a tumor behind the right upper eyelid 2 years before. Migration of MIRAgel^® appeared to be the cause of the tumor. Findings：Corrected visual acuity was 0.1 right and 1.2 left. The right eye was directed downward. A piece of MIRAgel^® was exposed in the lid opening showing rupture and calcium deposit. Computed tomography showed its diameter to be 10 mm. Superior rectus muscle could not be identified during surgery to remove the buckling material. Diplopia in the primary position improved after surgery. Conclusion：This case illustrates that early removal is recommended after exposure of MIRAgel^® after retinal detachment surgery.
