Abstract Purpose:To report both the time of appearance of ganglion cell complex(GCC)thinning and its thinning rate after posterior cerebral artery infarction.

Method:Four patients with homonymous hemianopia were examined using optical coherence tomography(OCT). A 9- and 4.5-mm circle centered on the macula was divided vertically and horizontally into 8 sectors. Mean GCC thickness was calculated at 4 sectors(inner sector:1.5 to 4.5 mm, outer sector:4.5 to 9 mm)on the hemianopia side. The GCC thinning rate was evaluated using the follow-up mode(NIDEK)from the onset of occipital damage.

Result:GCC thinning was detected from the cerebral infarction to 2 years and 5 months thereafter(mean:about 9 months). GCC thinning appeared on both sides almost concurrently, but in 1 case appeared about 2 years later than on the other ocular side. GCC thinning was marked on the nasal retinal side and particularly at the inner sectors. In 1 case with glaucoma GCC thinning could also be detected because there was no overlap with glaucomatous changes.

Conclusion:Retinal GCC thinning began within almost 1 year after cerebral infarction and 2 types of thinning were found. The nasal retinal GCC and central GCC were likely damaged.
