網膜細静脈瘤様所見を呈した網膜血管腫の1例

A case of retinal hemangioma simulating retinal venous macroaneurysm

要約　目的：網膜細静脈瘤様の所見を呈した後天性網膜血管腫の1例の報告。

症例：65歳の女性が健康診断で右眼底に白斑を指摘され受診した。53歳時，右内頸動脈瘤（未治療）を指摘されたが，脳内や全身には異常はなかった。58歳時，右網膜上膜を当院にて指摘されたが，高血圧症や網膜静脈分枝閉塞症，糖尿病などはなかった。

所見と経過：右眼黄斑部下方に硬性白斑と下耳側網膜静脈遠位部に橙色腫瘤を認めた。光干渉断層血管撮影（OCTA）では網膜静脈に茎部を有する瘤が検出され，瘤内部にも血流を認めた。フルオレセイン蛍光造影とインドシアニングリーン蛍光造影（IA）でもOCTA同様網膜静脈に接する瘤が描出された。当初網膜細静脈瘤と思われたが，IA動脈相早期において瘤が描出されていた。OCTA像を再検討したところ網膜動脈からの細動脈が多数瘤内へ流入する所見を認めた。内頸動脈瘤以外全身所見はないため未成熟な後天性網膜血管腫と診断した。

結論：網膜細静脈瘤様所見を呈した網膜血管腫を認めた。今後典型的な流入，流出血管を有する網膜血管腫を形成するか経過観察が必要である。

Abstract　Purpose：To report a case of retinal hemangioma simulating retinal venous macroaneurysm

Case：A 65-year-old woman with a history of epiretinal membrane was referred to us to examine her right eye with a hard exudate. A tumor was detected at the distal site of right retinal vein in the inferio-temporal quadrant. Optical coherence tomography angiography（OCTA）showed a tumor with a stalk budding from a retinal vein and blood flow in it. It resembled retinal venous macroaneurysm but indocyanine green angiography revealed this tumor at the early arterial phase. This tumor was diagnosed as acquired retinal hemangioma by re-analysis of OCTA. Many retinal arterioles drained into this hemangioma.

Conclusion：Retinal hemangioma similar to retinal venous macroaneurysm was reported. This hemangioma must be carefully followed up whether it will grow into typical hemangioma with feeder vessels or not.
