Abstract Purpose: To report a case of Listeria endophthalmitis diagnosed 6 days after onset with favorable outcome following prompt and conservative treatment.

Case: A 71-year-old woman was referred to us for pain and injection in the left eye since 3 days before. She had been receiving peroral prednisolone and methotrexate for rheumatoid arthritis in the past 20 years.

Findings and Clinical Course: Best-corrected visual acuity was 1.5 right and 0.04 left. The left eye showed findings of anterior uveitis with hypopyon. The vitreous and ocular fundus could be examined. The findings deteriorated after conventional treatment for anterior uveitis. Liseria monocytogenes was isolated from the aqueous aspirated 6 days after onset. Systemic treatments with antibiotics including penicillinwas followed by improvement of endophthalmitis with left visual acuity of 0.7 three weeks after onset.

Conclusion: Early diagnosis and prompt systemic treatments seemed to have resulted in favorable outcome in the present case of Listeria endophthalmitis.
