線維柱帯切開術既往眼に対する眼内内視鏡下スーチャートラベクロトミーの長期成績

Long-term outcome of suture trabeculotomy ab interno with an endoscope for eyes after trabeculotomy ab externo

要約　目的：眼外からの線維柱帯切開術が有効であった緑内障で，数年後に眼圧が再上昇した症例における，眼内内視鏡下スーチャートラベクロトミーの有効性について検討した。

対象と方法（症例）：10人10眼（男性6例，女性4例）。初回手術時の年齢は，75.4±11.1歳であった。疾患の内訳は，落屑緑内障9眼，原発開放隅角緑内障1眼であった。初回手術として8時方向から線維柱帯切開術を行い眼圧下降が得られたものの，数年後に眼圧再上昇を認め，眼内内視鏡下にスーチャートラベクロトミーを初回手術以外の部位に追加施行した緑内障眼を対象とした。線維柱帯切開術以外の眼科手術歴のないものが1眼，白内障手術の既往があるものが4眼，初回手術時に白内障手術を併施したものが4眼，スーチャートラベクロトミーに白内障手術を併施したものが1眼で，いずれもレーザー治療の既往はなかった。角膜混濁を1眼に認めた。

結果：線維柱帯切開術が有効であったものの，平均で約3年後に眼圧再上昇をきたした10例10眼において，眼内内視鏡下スーチャートラベクロトミーを追加施行したところ，術前24.6±4.3mmHgから術後半年で11.1±5.5mmHgに眼圧が再下降した。

結論：眼内内視鏡下スーチャートラベクロトミーは，線維柱帯切開術後の再手術として有効であった。

Abstract　Purpose：To examine the efficacy of suture trabeculotomy ab interno with an endoscope a few years after trabeculotomy ab externo.

Cases and Method：This retrospective study was made on 10 eyes of 10 patients who had glaucoma. The series comprised 9 exfoliation glaucoma and 1 primary open-angle glaucoma. There were 6 males and 4 females. The average age was 75.4±11.1 years. 10 patients received trabeculotomy ab externo around 8 o'clock direction and got reduction of intraocular pressure. But a few years later, intraocular pressure increased again, therefore they received suture trabeculotomy ab interno with an endoscope except the place where received trabeculotomy ab externo.

Findings：Suture trabeculotomy ab interno with an endoscope reduced the intraocular pressure of 10 eyes.

Conclusion：Suture trabeculotomy ab interno with an endoscope is a useful choice.
