Abstract Purpose:To present a 24-year review of 12 cases of ocular inflammation associated with inflammatory bowel disease.

Cases and Method:Ocular inflammation with inflammatory bowel disease was diagnosed in 12 patients at the Department of Ophthalmology, University of Tokyo Hospital from 1996 through 2018. The series comprised 4 males and 8 females. Three cases were unilaterally and 9 cases were bilaterally affected. The age ranged from 35 to 77 years, average 48.7±13.4 years.

Findings:The primary diseases included ulcerative colitis in 10 cases, and Crohn's disease in 2. Before manifestation of eye diseases, 8 patients had been treated by 5-aminosalicylic acid, 4 had received bowel surgery, and one had received intravenous infliximab. The interval between onset of bowel disease and ocular inflammation ranged from 9 to 34 years, average 19.1±7.9 years. The ocular involvement included anterior uveitis in 11 cases and episcleritis in one. Secondary glaucoma developed in one. Nine cases showed relapse of ocular inflammation. There was no case of visual loss due to ocular inflammation at the last observation.

Conclusion:Ulcerative colitis was the most frequent ocular involvement related to inflammatory bowel disease. Bilateral recurrent anterior uveitis was typical for ocular involvement. Visual outcome was generally favorable in spite of frequent relapses.
