Abstract Purpose:To report a child who developed bilateral macular hemorrhage following surgery under general anesthesia.

Case:A 6-year-old girl received adenoidectomy and tonsillectomy under general anesthesia that lasted 4 hours and a half. She was seen by us for central scotoma in both eyes 5 days later.

Findings and Clinical Course:Corrected visual acuity was 0.5 in either eye. Both eyes showed preretinal hemorrhage of 2 disc diameters across in the macular area. Fluoescein angiography showed blocked fluorescence in the area of hemorrhage. Goldmann perimetry showed central scotoma in both eyes. The hemorrhage was spomtaneously absorbed 2 months later with visual acuity of 1.2 in either eye.

Conclusion:Preretinal hemorrhage in this child is interpreted as an instance of Valsalva hemorrhagic retinopathy induced by anger and crying for several hours after surgery.
