Abstract Purpose:To report the outcome of brain surgery in a patient of meningioma with walking paralysis examined by optical coherence tomography(OCT)and Humphrey field analyzer(HFA).

Case:A 70-year-old female presented with impaired vision in both eyes.

Findings and Clinical Course:Intraocular pressure by non-contact tonometer was 14 mmHg in the right eye and 13 mmHg in the left eye at the first examination. Visual acuity of the left and the right eye was 0.8 2 months after first examination. Intraocular pressure and visual acuity remained unchanged during the whole clinical course. HFA showed MD values of −3.33 dB in the right eye and −2.46 dB in the left 8 years and half after the first examination. She staggered 13 years and 5 months after first examination, and we found thinning of thickness and defect of retinal nerve fiber layer(RNFL)in retina by OCT a month later. Walking paralysis happened next month, and MD values decreased up to −10.26 dB in the right eye and −8.7 dB in the left. Superior quarter visual field defect of nasal side in the right eye and temporal side in the left was detected. Magnetic resonance imaging(MRI)showed brain tumor in the temporal lobe and extensive edema. After the tumor was removed by surgery, diagnosis by histological observation was meningioma. Walking paralysis recovered, superior visual field defect of both eyes by HFA improved, and MD values decreased up to −1.75 dB in the right eye and −2.85 dB in the left.

Conclusion:OCT was helpful for the patient with systemic disorders that was suspected as intracranical disease.
