Abstract Purpose:To clarify the difference in micro-structure of trabeculum between non-glaucoma and glaucoma eyes, using adaptive optics fundus camera.

Subjects and Methods:Subjects were 12 eyes of 6 non-glaucoma cases(52.2±21.6 years), and 12 eyes of 6 glaucoma cases(60.0±26.0 years). An adapter for the anterior ocular segment OCT was attached to the adaptive optical fundus camera, and anterior chamber angle including the trabeculum was photographed. From the obtained images, the conspicuous irregularity, meshwork-like degree of the trabecular surface, was quantified as “coefficient of variation”. Multiple regression analysis was performed with the coefficient of variation of trabeculum as a target variable. Seven factors were selected as explanatory variables:glaucoma presence, age, sex, IOL presence, intraocular pressure, MD value, and presence of pseudoexfoliation.

Results:The trabecular surface in young non-glaucoma eyes showed a meshwork appearance, while the trabecular meshwork surface in elderly glaucoma eyes tended to be almost flattened and plate-like. Older aged non-glaucoma eyes and younger glaucoma eyes showed intermediate appearance. Coefficient of variation showed high values in young non-glaucoma eyes, while low values in elderly glaucoma eyes. Multiple regression analysis showed that glaucoma eyes(p<0.01)and age(p<0.01)reduced coefficient of variation significantly.

Conclusion:By combining an adaptive optical fundus camera with an adapter for the anterior ocular segment OCT, we were able to obtain images of fine surface structure of trabecular meshwork in the living eye. In glaucoma, the structure of trabecular meshwork may have collapsed and may have a pathological condition similar to age-related changes.
