Abstract Purpose:We performed cluster analysis using data of blood tests to classify patients with ocular sarcoidosis into groups, and compared the clinical features among groups.

Subjects and Methods:We analyzed 36 patients diagnosed with ocular sarcoidosis at the Department of Ophthalmology, Tokyo Medical University Hospital between January 2015 and March 2019. Cluster analysis was performed using the values of peripheral blood leukocyte count, CH50, C-reactive protein(CRP), serum soluble interleukin 2 receptor(sIL-2R), and angiotensin converting enzyme(ACE)from the data of blood tests conducted at diagnosis.

Results:According to cluster analysis, the patient population was classified into five groups:① elevated CH50, CRP, sIL-2R and ACE group, ② elevated ACE group, ③ elevated CH50 group, ④ unclassifiable group, and ⑤ reference values group. In these five groups, we compared visual acuity at the first visit, final visual acuity, intraocular pressure, treatment with oral steroids, history of surgery, presence of macular edema, presence of vitreous opacity, and presence of bilateral hilar lymphadenopathy(BHL). A significant difference in the presence or absence of BHL was found among five groups. Next, the subjects were divided into two groups according to the presence or absence of BHL, and blood test data were compared between two groups. The analysis showed that elevated serum sIL-2R and ACE concentrations were significantly related to the presence of BHL. Also, the probability of BHL was high when serum sIL-2R exceeded 838 U/ml or when serum ACE exceeded 18.6 IU/l at 37℃.

Conclusion:In ocular sarcoidosis, cluster analysis using data of blood tests identifies five distinct groups. Cluster analysis may be useful to predict the presence or absence of extraocular symptoms of sarcoidosis.
