新潟大学病院における過去12年間の急性網膜壊死症例の検討

A twelve-year review of acute retinal necrosis at Niigata University Hospital

要約　目的：急性網膜壊死（ARN）は高率に網膜剝離（RD）をきたし，視機能不良に至る症例が多い疾患である。RDを合併した例とRDを合併しなかった例で，臨床的経過・所見にどのような差があるのか，RDの原因となる臨床的所見や素因について比較検討した。検討には2015年の診断分類における臨床診断群も加えた。

対象と方法：対象は2006年9月〜2018年12月にARNを発症し，新潟大学医歯学総合病院眼科（当科）で初診・治療された患者20例22眼。うち確定診断群18眼，臨床診断群4眼であった。これらのうちRDを発症した例（剝離例）と発症しなかった例（非剝離例）に分け，比較検討を行った。

結果：両眼発症を20例中2例に認めた。原因ウイルスは水痘・帯状疱疹ウイルス15眼，単純ヘルペスウイルス3眼，PCR陰性4眼であった。12眼にRDを生じ，10眼はRDを生じなかった。年齢は，剝離例で50.3±18.7歳，非剝離例で69.0±16.6歳と，剝離例で有意に若年の傾向を認めた（p＝0.049）。剝離例は非剝離例と比較し，有意差はなかったものの，比較的病変の範囲が狭い例が多かった（p＝0.106）。剝離例と非剝離例の間の視神経乳頭所見・硝子体混濁・後部硝子体剝離の差はなかった。

結論：当科では，RD例が非剝離例と比較して有意に若年の傾向を認めた。

Abstract　Purpose：To present a 12-year review of acute retinal necrosis uveitis with particular attention to associated retinal detachment.

Method：This study was made on 22 eyes of 20 patients who were diagnosed with acute retinal necrosis during 12 years through 2018. It was unilateral in 18 cases and bilateral in 2 cases. The series comprised 13 male and 7 females.

Result：The age averaged 57.2±18.7 years. Culture of the aqueous showed varicella zoster virus in 15 eyes, herpes zoster virus in 3 cases, and was negative in 4 eyes. Eventual retinal detachment developed in 12 eyes and not in 10 eyes, with age averaging 50.3±18.7 years and 69.0±16.6 years respectively. There was no difference in findings of optic disc, vitreous opacity, or posterior vitreous detachment between eyes that developed retinal detachment or not.

Conclusion：Retinal detachment was more often in younger patients of acute retinal necrosis.
