Abstract Purpose:To report a case of facioscapulohumeral muscular dystrophy who received vitreous surgery for retinal vascular lesion simulating Coats disease.

Case:A 66-year-old male was referred to us for impaired vision in the right eye since 2 months before. He had been diagnosed with facioscapulohumeral muscular dystrophy 4 months before.

Findings and Clinical Course:Corrected visual acuity was 0.2 right and 0.3 left. Both eyes showed nuclear cataract. The right eye showed avascular area and neovascularization in the midperipheral and peripheral fundus. The findings simulated Coats disease. Fibrovascular proliferation with traction retinal detachment was present in the fundus area posterior to vascular arcades. The right eye was treated by panretinal photocoagulation followed by vitreous and cataract surgery. Visual acuity improved to 0.5 and the patient has been doing well for 10 months until present.

Conclusion:This case illustrates that due precaution is necessary during vitreous surgery for fibrovascular proliferation with traction retinal detachment in the posterior fundus.
