Abstract Purpose:To report a case of occipital infarction who was suspected of homonymous hemianopia but who failed to show remarkable visual field changed except by Amsler chart.

Case:A 68-year-old male presented with paracentral scotoma since two days before. He had been under treatment for systemic hypertension and lipemia.

Findings and Clinical Course:Corrected visual acuity was 0.8 in the right eye and 0.6 in the left eye. Clinical manifestations were strongly suggestive of homonymous hemianopia. Findings by static Humphrey perimetry(C30-2)were within normal limits. Magnetic resonance imaging(MRI)showed infarction in the right occipital lobe. The patient was referred to neurosurgical department. Static Goldmann perimetry showed no remarkable abnormality. Static Humphrey perimetry(C10-2)was also within normal limits. Examination by Amsler chart showed pathological findings. Three-dimensional cerebral angiography showed patent right occipitotemporal and parietooccipital arteries.

Conclusion:In the present case of infarction of occipital tip, conventional visual field testing showed no remarkable abnormalities except by Amsler chart.
