Abstract Background:Hydrodissection is an important procedure in cataract surgery. It may cause infusion misdirection syndrome, making continuation of surgery difficult.

Purpose:To report the outcome of hydroless subcapsular separation of the lens in 400 consecutive cases.

Cases and Method:This study was made on a consecutive series of 400 eyes that received cataract surgery with hydroless method. A 20 G cannula was inserted without touching the anterior or equator capsule of the lens. Without injecting any fluid, it was possible to separate the lens capsule from the nucleus from 2 to 10 o'clock position. Immediately after separation or fragmentation of the nucleus, it was attempted to rotate the nucleus.

Results:After separation of lens capsule, the lens nucleus could be rotated in 321 eyes(80.3%). After fragmentation of the nucleus, the nucleus could be rotated in further 74 eyes(18.5%). No weakness of the zonule occurred due to hydroless separation. There was no infusion misdirection syndrome throughout the present series of 400 eyes.

Conclusion:In the consecutive series of cataract surgery in 400 eyes, the lens nucleus could be rotated in 395 eyes(98.8%)following hydroless separation. No infusion misdirection syndrome occurred in the present series.
