白内障手術を行った38名に対する累進屈折力眼鏡の処方割合や矯正の調査

State of eyeglasses and use of progressive additional lens in 38 patients after surgery

要約　目的：白内障手術後（術後）患者の，矯正状態や眼鏡処方の状況を調査する。

対象と方法：対象は2015年6月〜2016年11月に両眼の白内障手術を行った38名（平均69.4±9.6歳）である。白内障手術前・術後の等価球面屈折値，眼鏡処方の割合，処方に至った日数をレトロスペクティブに調査した。

結果：38名の白内障手術前の等価球面屈折値は−1.31±4.35D（平均SE），術後平均SEは−1.13±1.54Dであり，有意差はなかった（t検定，p＝0.3418）。38名中27名（平均70.7±10.0歳）は，術後に梶田眼科にて眼鏡処方を行っていた。27名中26名は，累進屈折力レンズを処方しており，加入＋1.75Dが17名，加入＋2.50Dが9名で，手術から眼鏡処方までの平均日数は45日間であった。処方後に加入度数の変更を行ったのは26名中3名で，3名とも加入＋1.75Dから加入＋2.50Dに変更になった。当院で眼鏡処方を行っていない残りの11名の術後の矯正手段は，手術をした病院で作製した眼鏡を使用，術前に作製した眼鏡を使用，などがあった。

結論：術後，1か月以内に眼鏡処方に至った症例が半数であった。術前に累進屈折力レンズを使用している場合は，術後の最初の眼鏡処方において，高い加入度数でも問題なく使用できる可能性がある。累進屈折力レンズに慣れてくることにより，使用できる加入度数が変化していった。眼鏡作製の際は，度数を再調整するなどの選択肢が選べる，レンズの保証期間がある眼鏡店で作製するのが望ましい。

Abstract　Purpose：To report the correction and the situation of glasses prescription of patients after cataract surgery.

Methods：The subjects were 38 patients who received cataract surgery for both eyes from June 2015 to October 2016（mean 69.4±9.6 years old）. We investigated the mean spherical equivalent of objective refraction（mean SE）, the percentage of glasses prescription, and the days of glasses prescription after cataract surgery.

Result：The mean SE before cataract surgery was −1.31±4.35D and the mean SE after cataract surgery was −1.13±1.54D for 38 patients. No significant difference was observed（t-test, p＝0.3418）. 27 out of 38 patients（mean 70.7±10.0 years old）had prescribed glasses at Kajita Eye Clinic after cataract surgery. 26 out of 27 patients were prescribed progressive addition lens, 17 patients were prescribed a near add of ＋1.75D, 9 patients were prescribed a near add of ＋2.50D, and the mean number of days from the cataract surgery to the glasses prescription was 45 days. Among them, 3 out of 26 patients changed near add after prescription, in three cases changed from a near add of ＋1.75D to a near add of ＋2.50D. The correction after cataract surgery of the remaining 11 patients were using glasses prescribed in the surgical hospitals, or glasses prescribed before cataract surgery.

Conclusion：After cataract surgery, half of the cases received prescription of glasses within one month. In case that using progressive addition lens before cataract surgery, there is a possibility that high additional progressive addition lens can be used without problems in the first glasses prescription after surgery. Also, it became clear that the additional power that can be used will change by being used to the progressive addition lens. When ordering glasses, it is desirable to order them in the optical shop that has a guarantee period for glasses, with options such as readjusting the glasses.
