Bacillus cereus菌による重篤な外傷性眼内炎の1例

A case of severe traumatic endophthalmitis caused by Bacillus cereus

要約　目的：外傷性角膜穿孔後に急速にBacillus cereus眼内炎に至った1例の報告。

症例：71歳の男性が，左眼を竹で突き，翌日近医を受診，角膜潰瘍の診断にて紹介され受診した。左眼視力は手動弁で，多量の眼脂が認められ，角膜潰瘍部は穿孔しており，角膜は全体的に強い浸潤と浮腫で混濁していた。直ちに，細菌性と真菌性の両方の可能性を考慮して治療を開始した。2日後，角膜擦過物の培養検査からBacillus cereus菌が検出されたため，感受性のある抗菌薬へ変更を行った。角膜混濁が改善した11日後に保存角膜によるパッチ移植術，白内障手術，前部硝子体切除を施行した。現在，角膜は混濁が残存するものの，矯正視力は0.05まで改善している。

結論：急激に進行する外傷性角膜潰瘍・眼内炎はBacillus cereus菌も疑い，早急に感受性のある抗菌薬の治療を開始する必要がある。

Abstract　Purpose：To report a case of severe Bacillus cereus endophthalmitis following traumatic corneal perforation.

Case：A 71-year-old male was injured in the left eye by a bamboo stick while working in the farm. He was diagnosed with corneal ulcer the following day and was referred to us. Visual acuity in the left eye was hand motion. The left eye showed diffuse cloudiness and infiltration with impending perforation, and massive discharge. The findings suggested bacterial or fungal infection. Bacillus cereus was cultured from the eye discharge 2 days later, necessitating treatment with sensitive antibacterial agents. Patch keratoplasty using preserved cornea was performed after improvement of corneal lesion 11 days later. He received, additionally, cataract surgery and vitrectomy. Visual acuity improved to 0.05 ten months after injury.

Conclusion：The present case illustrates that rapidly progressive corneal ulcer and endophthalmitis may be characteristic features of Bacillus cereus infection.
