Abstract Purpose:To report a case of severe Bacillus cereus endophthalmitis following traumatic corneal perforation.

Case:A 71-year-old male was injured in the left eye by a bamboo stick while working in the farm. He was diagnosed with corneal ulcer the following day and was referred to us. Visual acuity in the left eye was hand motion. The left eye showed diffuse cloudiness and infiltration with impending perforation, and massive discharge. The findings suggested bacterial or fungal infection. Bacillus cereus was cultured from the eye discharge 2 days later, necessitating treatment with sensitive antibacterial agents. Patch keratoplasty using preserved cornea was performed after improvement of corneal lesion 11 days later. He received, additionally, cataract surgery and vitrectomy. Visual acuity improved to 0.05 ten months after injury.

Conclusion:The present case illustrates that rapidly progressive corneal ulcer and endophthalmitis may be characteristic features of Bacillus cereus infection.
