Abstract Purpose:To evaluate clinically and theoretically the effect of misalignment of toric intraocular lenses(IOLs)on postoperative refraction.

Materials and Methods:Nine eyes in which toric intraocular lenses were obviously misaligned were involved. The actual refraction of toric IOL misalignment was compared with theoretical calculation derived from postoperative measurement of angle of misaligned toric IOL and keratometry, as well as with similarly-calculated refraction assumedly without misalignment.

Results:The mean IOL misalignment was 37.1 degrees±21.1(SD)(ranged from 13 to 81). The mean sphere at misalignment was 1.03±0.49 diopters(D)by subjective refraction and 0.86±0.76D by theoretical calculation, which were significantly(both p<0.01)hyperopic compared to that of assump tion without misalignment(0.18±0.55D). The mean cylinder at misalignment was 2.50±0.48D by subjective refraction and 2.14±0.66D by theoretical calculation, both of which showed significant(p<0.01)undercorrection of preexisting astigmatism compared to that of assumption without misalignment(0.80±0.50D). Subjective spherical equivalent at misalignment was similar to that by theoretical calculation(p=0.47)as well as to that of assumption without misalignment(p=0.48).

Conclusion:In addition to reduction in astigmatic correction, misalignment of toric IOLs induced hyperopic spherical change and astigmatism axial change.
