Abstract Purpose:To retrospectively evaluate axial misalignments in the use of new types of toric intraocular lenses(IOLs).

Methods:XY1AT3-7(HOYA, group XY1), ZCW150/225/300/375(Johnson & Johnson Surgical Vision, group ZCW), and SN6AT3-9(Alcon, group SN)were implanted in 70 eyes during cataract surgery performed in the period between November 2017 and December 2021, under the same examinations, surgeon, and surgical procedure. Axial positions of the IOLs were aligned using a guiding system, and absolute difference between the intended and postoperative positions were measured and compared between the groups.

Results:Mean misalignments were 4.5±4.8 degrees, 3.0±2.2 degrees, and 2.4±1.7 degrees in the groups of XY1, ZCW, and SN, respectively, and the misalignments of XY1 were larger than those of SN(p=0.016). There were 7, 1, and 0 eyes with misalignments of 10 degree or more, respectively.

Conclusion:Misalignments in the ZCW and SN groups were lesser than in the XY1 group. The haptic exploration of ground surfaces of ZCW group IOLs were demonstrated to be effective in reducing misalignment compared with the previous model. The SN group IOLs resulted in the least misalignment with no record of high misalignment.
