トラスツズマブエムタンシン投与で角膜障害をきたした1例

Corneal lesion in a case after systemic treatment with trastuzumab emtansine

要約　目的：トラスツズマブエムタンシン（T-DM1，カドサイラ^®）による治療後に角膜障害が生じた症例の報告。

症例：再発乳癌に対して化学療法中の75歳の女性が，2015年9月からT-DM1の投薬を開始された後に，両眼の視力低下を自覚し，11月に受診した。

所見と経過：受診時の矯正視力は右（0.2），左（0.2p）と低下していた。右眼に角膜上皮びらん，左眼に偽樹枝状病変を伴う角膜上皮障害がみられた。T-DM1の投与中止後に両眼の角膜障害は寛解した。

結論：T-DM1は角膜上皮障害が生じる可能性がある薬剤として注意が必要である。

Abstract　Purpose：To repot a case whe developed corneal epithelial lesion following treatment with systemic trastuzumab emtansine（T-DM1, KADCYLA^®）.

Case：A 75-year-old woman, who was referred to our clinic, presented with reduction of visual acuity. Two months previously, she had been undergoing treatment with T-DM1.

Findings and Clinical Course：Corrected visual acuity was 0.2 in either eye. The right cornea showed irregular epithelium in the superior sector. The left cornea showed pseudo-dendritic lesion. Withdrawal of T-DM1 was followed by improvement of corneal lesions with visual acuity of 0.5 right and 1.0 left ten months later.

Conclusions：This case illustrates that prolonged systemic treatment with T-DM1 may induce corneal epithelial lesions.
