生後1か月の乳児に初発したヘルペス角膜炎の1例

Herpetic keratitis in a baby one month after birth

要約　目的：生後1か月の乳児のヘルペス角膜炎の1例報告。

症例：生後20日頃から左眼の充血，眼脂が出現し，レボフロキサシン点眼にて眼脂は減少したが，角膜混濁・潰瘍所見を認め，当科を紹介され受診した。

所見：左眼角膜に樹枝状形態を伴う地図状潰瘍，実質の円板状浮腫，結膜充血を認めた。角膜擦過物のPCRにて単純ヘルペスウイルス（HSV）DNA陽性，血液検査にてHSV IgM高値から単純ヘルペス角膜炎と診断した。アシクロビル眼軟膏およびフルオロメトロン点眼に加え，アシクロビルの全身投与も併用し，所見は改善した。

結論：生後1か月の乳児にヘルペス角膜炎が発症した1例を経験した。アシクロビルの局所および全身投与による治療が有効であった。

Abstract　Purpose：To report a one-month-old baby who developed herpetic keratitis.

Case：A 38-day-old male baby presented with hyperemia and discharge in the left eye since 20 days before. He had been born by Caeserean section weighing 2,532 g. Corneal opacity and ulcer had developed after treatment with topical levofloxacin.

Findings and Clinical Course：The left eye showed dendritic keratitis with disciform edema in the corneal stroma. Polymerase chain reaction（PCR）of corneal scraping showed DNA positive for herpes simplex virus. The blood showed high titer of IgM for herpes simplex virus. The findings led to the diagnosis of herpetic keratitis. The corneal lesion disappeared after 2 weeks of treatment with topical and acyclovir with topical fluorometholone.

Conclusion：A one-month-old baby was diagnosed with herpetic keratitis. Treatment with topical and systemic acyclovir appeared to be effective.
