Abstract Purpose: To report two cases of Sjögren syndrome who developed bilateral dacryolith.

Cases and Findings: Two females, aged 71 and 75 years respectively, presented with swelling of the eyelid and lacrimal sac. They were using rebamipide ophthalmic solution for dry eye. Both cases developed unilateral orbital cellulitis and dacryosystitis that healed after systemic treatment with antibiotics. Magnetic resonance imaging(MRI)showed bilateral dacryolith. Surgically removed mass appeared as whitish material simulating soy pulp. Histological study showed several crystal formation, inflammatory infiltrates, and colonies of Actinomyces. Clinical symptoms improved after surgery.

Conclusion: The present cases suggest that dacryolith may have formed after accumulation of rebamipide in the lacrimal sac.
