涙点に囊胞様隆起を認めた1例

A case of cystoid protrusion of the lacrimal punctum

要約　目的：涙点に発生した囊胞の症例を報告する。

症例：83歳女性。約2年前からの両側の流涙を主訴に受診した。両側の下涙点閉鎖と右下涙点の囊胞様隆起病変があった。

所見：右下涙点閉鎖に対して涙点形成術と涙道内視鏡検査を施行した。囊胞様病変を切除すると内腔は粘液で満たされ，涙小管と連続していた。下涙小管水平部は開通していたが，内総涙点の閉塞があった。切除検体は病理組織学的に重層扁平上皮に裏装される真性囊胞と診断された。

結論：本症例では二重閉塞により涙小管水平部は閉鎖空間となっていて，粘液産生能を有する総涙小管が含まれていた。これが囊胞形成の要因になった可能性がある。

Abstract　Purpose: To report a case of cystoid protrusion of the lacrimal punctum.

Case: A 83-year-old female presented with epiphora in both eyes since 2 years before. She had noticed a tumor at the site of lower lacrimal punctum.

Findings: Both eyes showed occluded lower lacrimal punctum. The right eye showed, in addition, cystic protrusion of the lower lacrimal punctum. She received excision of cystic protrusion that simulated a tumor. Dacryoendoscopy showed occlusion of common lacrimal duct. The tumor was histologically diagnosed with cyst lined by squamous epithelium.

Conclusion: An enclosed space was present by the stenosis of punctum and occlusion of common lacrimal duct. Presence of the enclosed space and secreted mucus seemed to have produced the cystic protrusion of the lacrimal punctum in the right eye.
