黄斑浮腫を伴った高γグロブリン血症の2例

Two cases of hypergammaglobulinemia with cystoid macular edema

要約　目的：黄斑浮腫を伴う高γグロブリン血症の2例を報告する。症例：症例1はIgA-κ型の多発性骨髄腫の73歳男性。両眼の黄斑浮腫により視力が低下した。右眼にトリアムシノロンテノン囊下注射（STTA）を施行したが改善せず，左眼には硝子体切除＋STTAを施行した。内科的治療で血清IgA値が低下すると黄斑浮腫は軽減し，特に左眼視力が改善した。症例2は原発性マクログロブリン血症の58歳男性。右眼の黄斑浮腫により視力が低下した。血漿交換施行した後一時的に視力が改善したが，その後網膜萎縮をきたした。結論：高γグロブリン血症に合併する黄斑浮腫の臨床経過とOCT所見を報告した。本症では，原疾患の治療に加えて硝子体手術が有効である可能性がある。

Abstract.　Purpose：To report 2 cases of hypergammaglobulinemia with cystoid macular edema（CME）. Cases：Both were males aged 73 and 58 years respectively. One had IgA-κ-type multiple myeloma with CME in both eyes. Subtenon injection of triamcinolone failed to improve visual acuity in the right eye. The left eye received additional vitrectomy. Decreased serum level of IgA was followed by alleviation of CME with improved vision in the left eye. The other case had primary IgM-κ-type macroglobulinemia with CME and corrected visual acuity of 0.15 in the right eye. Plasmapheresis was followed by improved visual acuity of 0.7. It decreased to 0.1 four months later due to macular atrophy. Conclusion：These cases illustrate that CME may develop in hypergammaglobulinemia. Decrease in serum level of gammaglobulin may be followed by reduced CME and improved visual acuity. Vitrectomy may be effective for persistent CME.
