眼内レンズ挿入術における術中縮瞳・術後炎症に対するケトロラックトロメタミン点眼液の効果

Effects of topical ketorolac tromethamine on inhibition of miosis during surgery and on inflammation after intraocular lens implantation surgery

　プロスタグランディンズ合成阻害作用を有するケトロラックトロメタミン（KL）の水性点眼液を用い，眼内レンズ挿入術における術中縮瞳，術後炎症に対する効果を定量的に検討した。対象は水晶体嚢外摘出術＋後房レンズ挿入を行った老人性白内障で，基剤投与群10眼，0.1％KL投与群11眼，0.5％KL投与群11眼，計32眼で，各群とも術前は3,2,1時間，30分前，術後は1日3回点眼した。術前と水晶体皮質吸引後の瞳孔径，術後前房内炎症について検討した。術後炎症はレーザー・フレア・セル・メーター（FC1000®興和KK）により術後4週目まで測定し，フレアー値は牛アルブミン濃度に換算した。

　前房内蛋白濃度についてみると，0.5％群は0.1％群に対して有意差は認められなかったが低値を示し続け，基剤群に対しては1日目から2週目まで有意に低かった。0.1％群は基剤群に対して2日目から1週目まで有意に低かった。

　細胞数は，基剤群が最高値を示し続けたが，蛋白濃度ほど差はなかった。術中の縮瞳率は，0.1％群，0.5％群とも基剤群より低値を示した。

We evaluated the effects of 0.1% and 0.5% keto-rolac tromethamine ophthalmic solutions on inhibi-tion of miosis during surgery and of postoperative inflammation in 32 eyes which underwent cataract extraction with intraocular lens implantation. Eleven eyes received 0.5% solution, 11 eyes received 0.1% solution and 10 eyes received the vehicle alone. Instillation started 3 hours before surgery and was continued 3 times daily after surgery.

There was no significant differences in the pupil size during surgery among the 3 groups. When quantitatively evaluated with a newly developed flare-cell meter, we observed decreased numbers of cells in the anterior chamber throughout the 4-week postoperative period in eyes treated with 0.1 or 0.5% solution. The amount of protein concentra-tion in the anterior chamber was less during the 4-week postoperative period in eyes treated with 0.1 or 0.5% solutions. Mild corneal erosion developed in 1 eye each treated with 0.1 or 0.5% solution.
