眼内レンズ表面膜様物質の由来と細胞の観察

Origin and cellular component of membraneous coating on the intraocular lens

　術後の眼内レンズ前面をスペキュラマイクロスコープで観察した結果，133眼のうち61眼において，術後4日目から225日目までの間に，灰色の無構造な物質が観察され，その一部は，光の干渉によって，ニュートンリング（干渉縞）を生じていた。灰色の無構造な物質は，術後早期では斑状に認められるが，時間の経過とともに，癒合，拡大し，膜状を示しながら，眼内レンズ前面を覆うのが観察された。この経過は，細胞のみられない部位でも観察された。このことから，灰色の無構造な物質は，眼内レンズ表面にみられた細胞が，分泌したものではなく，前房中の物質が吸着したものであると考えられた。

We examined the anterior surface of intraocular lens in 133 eyes during the postoperative period by means of specular microscopy. All the eyes had undergone extracapsular cataract extraction with implantation of posterior chamber lens made of PMMA. The observed cells appeared to be his-tiocytes, fibroblast-like cells or foreign body giant cells. In 61 eyes in the series, we observed an amorphous coating filling the spaces between thefibroblast-like cells and histiocytes. The amor-phous coating started as deposits of gray sub-stances which gradually fused with one another to form a uniform layer. The coating was occasionally observed as interference fringes simulating Newton rings.

The formation of coating was a rather common event on implanted intraocular lenses. Presence of Newton rings seemed to be indicative of the pres-ence of a thin coating.
