角膜移植術を施行したHurler-Scheie複合型の組織学的検討

Histopathology of the cornea in a case of Hurler-Scheie compound

　ムコ多糖体代謝異常の中でも報告の少ないHurler-Scheie複合型に対し，全層角膜移植術を施行し，摘出角膜片の組織学的検討を行った。

　症例は9歳女児で，両眼角膜はびまん性に混濁し，4年間の経過観察中に視力は手動弁にまで低下した。左眼角膜移植の結果，術後1年経過した現在，移植片は透明で経過良好である。

　摘出角膜の組織学的検討で，角膜上皮，実質細胞，内皮細胞の細胞質に細線維顆粒状物質，膜様物質を含む多数の空胞が観察され，実質層間，デスメ膜には異常な長周期線維が認められた。またボーマン膜の破壊，実質コラーゲンの走行異常も観察された。以上より，Hurler-Scheie複合型では，ムコ多糖体代謝異常による影響が，細胞成分のみならずコラーゲン形成にまで及んでいることが考えられた。

A 9-year-old girl with Hurler-Scheie compoundwas referred to us for evaluation of ocular compli-cations.Both corneas showed hazy stroma. Thediffuse corneal cloudiness gradually increased withthe visual acuity reduced to hand motion bilaterallyduring the ensuing 4 years.We performed penetrat-ing keratoplasty in her left eye at 14 years of age.The excised cornea was submitted to histopath-ological studies.

Light microscopy showed alcian-blue positivegranules in epithelial cells, keratocytes.and endoth-elial cells. Most of the cellular elements werevacuolated by electron microscopy.These vacuolescontained electron-dense inclusions of variableshapes. Abnormal collagen fibers were observed,suggesting pathological metabolic process in thesynthesis of collagen fibers.
