前房にコレステリン結晶が充満した1例

A case of severe cholesterosis in the anterior chamber

　コレステリン結晶が完全に前房内に充満した，珍しいcholesterosis bulbiの1症例を経験した。当初は変性水晶体の脱臼の可能性を考え，疼痛を寛解せしめるために観血的治療を試みたが，その術中所見及び採取したサンプルの分析からcholesterosis bulbiの診断を得ることが出来た。

　サンプルを生化学的に分析したところ，コレステロール以外の前房中の有機成分の濃度も上昇していることがわかった。本例の発症機序は確定できなかったが，長期の網膜剥離に伴った，硝子体中あるいは網膜下へのコレステリン析出，虹彩の新生血管や破壊された血液眼関門からの血漿漏出などの要因が関与しているものと考えられた。

A 74-year-old male sought medical advice due to pain in his left eye of one month's duration. He had undergone unsuccessful retinal detachment surgery in the left eye 15 years before.

The anterior chamber in the affected eye was completely filled with white glistening crystals. The crystals were removed by paracentesis and proved to be cholesterol. The concentrations of triglyceride, phospholipid and protain in the aspir-ated aqeuous were several times higher than nor-mal values.
