原発性錐体骨コレステリン囊胞の1例

Primary cholesterol cyst in the petrous bone

Ⅰ．はじめに

　乳突洞や鼓室内に発生するコレステリン囊胞は手術や外傷による出血や慢性炎症の既往を伴うことが多く，これらの既往のない原発性コレステリン囊胞は稀な疾患である^1,2）。今回われわれは右変動性感音難聴と耳鳴，めまいを呈し，その原因が右錐体骨の骨破壊を伴った占拠性病変による右前半規管瘻孔および後半規管瘻孔と考えられ，精査目的に乳突洞削開を施行し，原発性錐体骨コレステリン囊胞との診断に至った症例を経験したので報告する。

　We report a case of 34-year-old female with primary cholesterol cyst in the petrous bone. She complained of right tinnitus and fluctuating sensorineural hearing loss. MRI and CT revealed a mass lesion with bone defect of the anterior and posterior semicircular canal in the right petrous bone. The transmastoid infralabyrinthine approach to the petrous apex was successful for adequate drainage with hearing preservation, and histological diagnosis was cholesterol cyst. The clinical features, radiological findings, and surgical approach are discussed.
