裂孔原性網膜剥離に対する手術後の一過性網膜深層変化

Transient patchy lesions in deeper retinal layers following surgery for rhegmatogenous retinal detachment

　裂孔原性網膜剥離に対する手術後，術後経過が良好であっても，斑状の網膜深層変化が一過性に生じることがある。これについて，過去8年2か月間に手術を行った裂孔原性網膜剥離214眼を検討した。斑状の網膜深層変化は，経強膜手術141眼中3眼，硝子体手術群73眼中1眼にあった。この斑状変化は，術前に網膜剥離が存在した範囲内に術後1〜3か月の時点で出現し，その後4〜5か月間に自然消退した。蛍光眼底造影では，3眼が過蛍光，1眼が低蛍光を呈した。この違いから，病態は必ずしも1つではないと考えられた。

Transient patchy lesions in deeper retinal layers may occasionally develop following successful surgery forrhegmatogenous retinal detachment. We evaluated this feature by reviewing 214 eyes that were operated for rhegmatogenousretinal detachment during the past 8 years and 2 months. Patchy lesions were found in 3 out of 141 eyes that receivedscleral buckling and in 1 out of 73 eyes that received vitrectomy. The lesions appeared 1 to 3 months after surgery in the former area of retinal detachment and spontaneously disappeared 4 to 5 months later. Fluorescein angiography showedhyperfluorescence in 3 eyes and hypofluorescence in one eye. This finding seems to show that multiple pathologicalchanges may be involved in this patchy-pattern lesion.
