シリコンチューブ留置による涙道閉塞の治療

Silicone tube intubation for treating lacrimal obstructions

　ブジーまたは注射針で開放可能な26例30眼の涙道閉塞にシリコンチューブ留置を行い，4例4眼を除き良好な結果が得られた．有効であった症例ではチューブ留置後2〜3日以内に症状の消失ないし改善がみられた．

　本方法の適応は，注射針またはブジーで一時的に開放可能な涙道閉塞であるが，主として，（1）小児の先天性および後天性鼻涙管閉塞，（2）涙小管閉塞（涙小管損傷を含む），（3）涙点閉鎖，（4）涙嚢炎を伴わない鼻涙管閉塞，（5）涙嚢内腔癒着，狭小化（DCRに併用），（6）（1）〜（5）の合併例が挙げられる．

We treated a consecutive series of 26 patients, 30 eyes, with lacrimal obstructions by implanting silicone tube of Crawoford, either as a primary procedure or in combination with dacryocystorhinostomy. Almost all the cases had been treated in vain by conventional means.

The therapeutic results were judged as success in 26 eyes, 23 patients, including 2 eyes with combined block-ages in the canaliculus, lacrimal sac and nasolacrimal duct. In these two eyes, dacryocystorhinostomy and tubing were performed as a combined surgery, with the knots of the tube placed in the re-formed lacrimal sac.
