涙道閉塞症に対するシリコンチューブ留置術

Silicone tube intubation for lacrimal obstruction

　涙道閉塞症40例47眼に対してシリコンチューブ留置術を行った。自覚症状が改善し，かつ涙道通水試験にて通過した症例が47眼中32眼（68％）あり，改善しなかった15眼のうち5眼は，抜去後再閉塞をきたした症例で，全例3か月以内にチューブを抜去したものであった。他の10眼はチューブ留置中より，涙道通水試験の改善はなく，うち6眼は高度の慢性涙嚢炎あるいは副鼻腔炎の手術既往歴があった。本法は慢性の炎症に起因する高度の癒着を伴う症例を除く涙道閉塞症に対して第一選択としてよい方法と考えられた。

We treated 47 eyes with obstructed lacrimal passage by intubation of silicone tube in the naso-lacrimal duct. The tube was removed after 3 months. The treatment was successful in 32 eyes, 68%. The failure was due to reobstruction of the lacrimal passage after removal of the tube in 5 outof the 15 eyes. The remaining 10 eyes showed obstruction of the silicone tube before removal. Persistent dacryocystitis or paranasal sinusitis was present in 6 of the 10 eyes. The present method is recommended as a first choice to relieve obstructed lacrimal passage excepting cases with chronic inflammation and cicatrization.
