The international classification of acute retinopathy of prematurity (ROP) is reviewed in a historical per-spective and is compared with the 1982 version of the classification by the Japanese Joint Committee for the Study of ROP.

The international and Japanese classifications are basically similar so that it is easy to convert the staging from one to the other, provided the location, extent and degree of vascular lesions are recorded on the standard-ized scheme.

The concept of "+" disease in Type II retinopathy differs in the two systems. In the Japanese classification, the "+"disease indicates a stage of nontreated case of Type II retinopathy. It is stressed that the effect of treatment may not be expected unless the disease is diagnosed early and intervened before the "+" signs develop.

We tentatively evaluated a nation-wide series of cases with acute ROP using the international and Japanese classifications. No problems were encountered in converting the staging between the two systems.
