A 9-year-old boy with acute lymphatic leukemia of 4 years' duration developed hypopyon and secon-dary glaucoma in his right eye. Systemic predni-solone and radiation of the anterior ocular segment caused the hypopyon and glaucoma to subsisde only temporary, necessitating trabeculectomy and aspira-tion of the anterior aqueous. The right eye was obtained for autopsy one year later.

The hypopyon aspirated during surgery was com-posed of accumulated leukemic cells, ranging those during the stage of mitosis to decomposed ones. Some of the latter were phagocytized by macro-phages. In the autopsied eyeball, leukemic cells infiltrated the uvea, trabecularmeshwork and the optic nerve. There were free cells in the anterior and posterior chambers. While the retina was not infiltrated by leukemic cells, the rod and cone outer segments were undergone extensive degeneration.
