サルコイドージス長期観察例に対する血清学的検討—第1報細胞性免疫について

Follow-up studies of cellular immunity in the patients with sarcoidosis

　長期間観察しえた眼症を有するサルコイドージス患者32名の免疫異常を検討し，以下の結果を得た。

（1） PPD値は，発症とともに陰性化し，回復につれて陽性化する。眼外症状の消退後も眼症が存在する間は，陰性を持続していた。

（2）活動期には末梢血でTγ（％）の低下，OKT 4陽性細胞の低下，　OKIa陽性細胞の増加が有意に認められた。この傾向はPPD陰性検体に著明で，　Tγ（％）の低下は眼症のみの遷延化例で多く認められた。

（3）血清免疫グロブリン値はIgMのみが高値であった。

（4） ACE値は肺病変の活動性を敏感にとらえるが，活動性眼病変のみでは高他を示さなかった。

A series of 32 patients with ocular sarcoidosis were evaluated as to cellular immunity. The dura-tion of sarcoidosis ranged from 1 to 118 months. All the patients were tested as to the following items at each follow-up visit : PPD skin responses, blood lymphocyte count, T and B cells, IgG･FcR⁺ T-cells (Tγ cells), serum ACE level, and reactivity with monoclonal antibodies (OKT 3, 4, 8 and OKIa) to surface antigens of T lymphocytes in the peripheral blood.

The PPD skin response was negative in all the patients with active disease process even in the ab-sence of active extraocular lesions.
