硝子体手術後のinternal tamponade—Gas, silicone oil, gas-silicone oil exchangeの比較検討

Internal tamponade after vitrectomy. Comparative study of the results of gas, silicone oil and gas-silicone oil exchange

　重症例に対する硝子体手術の補助手段として，internal tamponadeは重要な位置を占めている。我々は75例80眼に対し，gas （airまたはSF₆）18眼，silicone oil 48眼，gas-silicone oil exchange 14眼のinternal tamponadeを施行した。Gas群77.8％，　siliconeoil群87.5％，　gas-silicone oil exchange群57.1％の復位率を得たが，復位率と視力予後とは一致しなかった。しかし，silicone oilの導入により，従来gasのみでは復位不能であった症例も治療可能となり，症例に応じたinternal tamponade素材の選択により，治療成績は飛躍的に向上した。

The internal tamponade was performed for 80 eyes in 75 cases, in which gas (air or SF 6) was in-jected in 18 eyes, silicone oil was injected in 48 eyes, and gas-silicone oil exchange was performed for 14 eyes.

Although the retinal reattachment rate was high in our results (77.8%A, in gas-injected group, 87.5% in silicone oil-injected group and 57.1% in gas-sili-cone oil exchange group), there was no coincidence between the retinal reattachment and the visual recovery.

The use of silicone oil is effective to save the eyes which are not possible to treat even with the use of the gas.
