単純型糖尿病性網膜症における網膜機能の多角的検討

Layer-by-layer analysis of retinal functions in simple diabetic retinopathy

　インスリン非依存性糖尿病患者に網膜機能の層別検討を試みた。検査項目は網膜電図律動様小波，眼球常存電位の高浸透圧反応，青の感度，transient tritanopia effectおよびvitreous fiuorophotometryである。すべての検査で異常の程度は網膜症の進行と平行する結果を得た。また，検眼鏡所見で点状出血を認めない時期に，すでに網膜色素上皮層の機能異常を示すにもかかわらず，内層の機能異常が認められない症例が存在した。さらに，この時期では内層機能の異常の強いものと軽いものの間に外層機能の障害に差を認めなかった。このことから網膜の内層と外層に異常をきたすメカニズムに相異があるのではないかと推測した。

We studied the dysfunctional layer of the retina in simple diabetic retinopathy, particulary in its early stage. Thirty noninsulin-dependent diabeticpatients were examined. All had good visual acuity and were free from high myopia. Patients were ex-amined by : electroretinogram (ERG), hyperosmo-larity response of the standing potential, psychophy-sical measurement of the Transient Tritanopia Effect and blue sensitivity, and vitreous fluoropho-tometry. An analysis of layer-by-layer functions in the retina was made based on combined evalua-tion of these tests.
