透析患者における白内障手術について

Cataract surgery in patients undergoing hemodialysis

　透析患者7例8眼に白内障重術を行い次の結果を得た。

（1）8眼中7眼は術中術後何ら合併症の発生をみなかったが．1眼に術後創癒合不全，硝子体瞳孔ブロックによる続発性緑内障およびアセタゾラマイド蓄積によると思われる意識障害の発生をみた。

（2）術前および術後の透析の時期と白内障手術の合併症の発生は相関しないと思われた。

（3）術後視力は糖尿病性網膜症を有する2眼を除けば平均0.9の矯正視力を得た。

　透析患者に対する白内障手術は全身管理を第一を第一とし，良好なコントロールのもとに手術を行えば一般患者に対する白内障手術と同様安全な手術が行えると思われる。

We evaluated the outcome of intracapsular cat-aract surgery in a consecutive series of 7 patients (8 eyes) who had been under hemodialysis treat-ment for chronic renal insufficiency of variable causes in the past 5-year period.

There were no complications during or after sur-gery in 7 out of the 8 surgeries. In the remaining one case, delayed wound healing and secondary glaucoma due to pupillary block resulted. In this particular patient, postoperative drowsiness also occurred apparently due to accumulation of aceta-zolamide.
