網膜静脈閉塞症に対する治療法—特に線溶療法と光凝固療法の応用について

Evaluation of fibrinolytic therapy and photocoagulation in retinal vein obstruction

　網膜静脈閉塞症に対し，線溶活性剤urokinase初回量10万単位を使用し，その後光凝固療法を併用してその治療効果をみた。

（1）10万単位投与にて線溶亢進状態を示すPT, aPTTの有意の延長があり，血液粘稠度も低下した。

（2）光凝固療法併用にて視力改善率75.9％，　0.6以上の症例が53.1％と良好な結果を得た。

　以上より，urokinaseをもってする線溶療法は有効な治療法であるが，それのみで改善の得られない例ではすみやかに適応を決定し，光凝固療法を比較的早期に併用することにより，視力の予後に関しても良好な結果が得られることが判明した。

Effect of combined therapy with systemic fibri-nolytic agent and photocoagulation was evaluated in 145 patients with retinal vein obstruction (23 cases of central retinal vein obstruction and 122 cases of branch vein obstruction).

Fibrinolytic therapy with urokinase was started with the initial dose of 100,000 IU as intravenous drip infusion. It was given daily with decreasing the dosis for an average of 7.6 days and 418,000 IU as total dosis.

While the treatment induced improvements in PT, aPTT and blood viscosity values in the ma-jority of cases, visual or circulatory disturbances resulted in some but not in all the cases.
