境界型GTTと網膜静脈閉塞症との相関について

Etiologic role of borderline GTT in obstruction of the retinal vein

緒　言

　予防医学の考えが普及するに伴い，糖尿病や高血圧症等のいわゆる成人病が，早期に発見される機会が多くなつた今日，眼科臨床的にもこれら疾患の初期例に遭遇する機会は増加してきている。これらの症例の中の一群として，境界型GTT症例が多数存在する事は周知である^1〜4）。境界型GTTとは，50g糖負荷試験にて，日本糖尿病学会の勧告値により^5），正常型にも糖尿病型にも属さない，ちようど中間のパターンを示す一群であり，糖尿病近縁疾患であると考えられている（表1）。

　しかし，本症の詳細すなわち，本症がはたして糖尿病に移行してゆくか否かさえ，今日なお明らかにはされていない^1,6〜9）。また，人間ドツクなどで遇然発見された本症患者は，その大部分がfollow upされずに放置されているのが現状であろう。

Correlation between obstruction of the retinal vein and borderline glucose tolerance test (GTT) was studied in this report.

Main causes of central or branch retinal vein obstruction in our 68 cases were hypertension (16), arteriosclerosis (14), diabetes mellitus (4), and unknown (26). Of these 26 cases of unknown cause, 15 cases revealed borderline GTT.Dense and massive hemorrhages and a tendency for delayed absorption of the hemorrhage were common ophthalmoscopic features in this group.
