ジフェニールビダントイン，フェノバルビタールにより多彩な眼症状を示した1症例

Complex ocular manifestations of long-term diphenylhydantoin and phanobarbital. A cas ereport

　ジフェニルヒダントイン（DPH），フェノバルビタール（PB）の長期内服の症例において，眼瞼下垂，瞳孔異常，視力障害が生じた1例を経験した。眼瞼挙筋筋電図・瞳孔点眼試験より，眼瞼下垂は重症筋無力症およびHorner症候群，瞳孔異常はHorner症候群と同様の機序によるものと考えられた。眼底はbull's eye様変化を示しており，螢光限底撮影・網膜電気生理学的検査・暗順応反応より，網膜色素上皮の広範な障害が推測された。薬剤の血中濃度は高値を示しており，薬剤内服中止により眼瞼下垂・瞳孔異常は消失したが，眼底変化は不可逆であった。

A 43-year-old male developed bilateral ptosis and visual disturbance since 3 years before. He had been under continuous treatment for epilepsy with sys-temic diphenyl hydan toin ( total dosis : 2,200 gr) and phenobarbital (total dosis : 6,200 gr) for the past 20 years.

The ptosis was diagnosed as combined myasthe-nia gravis and Homer's syndrome based on electro-myography of the levator muscle and pupil reac-tions for epinephrine and tyramine. The fundus manifested bull's eye macula bilaterally. Diffuse im-pairment of retinal pigment epithelium was sug-gested by fluorescein angiography, electroretino-graphy and dark adaptation.
