Three cases of internal carotid artery occlusion with ipsilateral Horner's syndrome are reported. Pupillary drug test in one case indicated that Horner's syndrome was caused by the disturbance of post-ganglionic neuron. Superior cervical gan-glions in two cases were examined and found to be intact.

Sympathetic nerve fibers around the occluded carotid artery might be injured by stretching, ischemia or other mechanism resulting in Horner's syndrome.
