第一次硝子体過形成遺残に関する統計的考察

Persistent hyperplastic primary vitreous : An analytical study of 63 cases

　第一次硝子体過形成遺残の研究のために，その臨床的実態を明らかにする目的で，慶大眼科において1973年7月より1981年6月までの8年間に経験した63症例，77眼について統計的観察を行なった。その結果は次のごとくである。性別は男子に多く，女子の約2倍にみられ，片眼性の症例は77.8％，両眼性は22.2％であった。Type別にみるとAnterior Type 44.4％，　Intermediate Type 28.6％，　Posterior Type 22.2％であった。

　超音波検査をAnterior Typeの症例に施行した結果，従来Anterior Typeとされているものの37.5％にIntermediate Typeが存在していることを明らかにした。ERG, VEPの検査によりAnterior Typeにおいても網膜，視路の機能障害を高率に認めた。このことは，治療，予後にはもちろんのこと本症の病態を研究する上に注目すべき事実である。従来Anterior Typeは，その大部分は緑内障を併発し眼球癆におちいると報告されてきたが，今回の観察では，それぞれ3例にすぎず，また網膜芽細胞腫と誤診し眼球摘出をした症例もなかった。

We evaluated 63 cases with persistent hyper-plastic primary vitreous (PHPV) in order to eluci-date the clinical entity of this disease There were 40 males and 23 females. 49 cases (78%) were bilateral and 14 cases (22%) were unilateral. In the 63 cases of PHPV 28 cases (44%) were the anterior types, 18 cases (29%) were the interme-diate types and 14 cases (22%) were the posterior types. In our present series, the intermediate type was the significant greater frequancy than that in previous reports. Our present series was charac-terized by the significant greater frequency of intermediate type than reported in literature.
