視神経乳頭部の先天異常について—朝顔症候群と乳頭部第一次硝子体過形成遺残

Congenital Optic Disk Anomaly:Morning Glory Syndrome or Persistent Hyperplastic Primary Vitreous at the Optic Nerve Head

緒言

　日常の眼科診療において，従来の成書に記載されている分類や病名にあてはまらない症状を示すものに遭遇することがある。ことに，先天性眼異常においてはそのことが多い。文献を調べてみると，その類似症候を示すものにつきあたることはあるが，まちまちの名称がつけられている。また，その症状が経過とともに変化するものがあり，その実体を捉えるには，数年の経過をみる必要のあることもある。このような分類，名称の不明，あるいは同一疾患を別個の名称でよばれているようなものは，これらの症例を丹念に集め，その特徴的な所見を明らかにし，病理組織学的の検索，さらには原因を究明し，名称，分類を確立してゆく必要がある。著者らは，すでに2,3の先天異常については，以上の研究方法によつて新しい病名，症候群を発表してきたが，今回は，視神経乳頭部の異常のなかで，小児の網膜剥離との関連において注目されており，Morning glory syn—drome Posterior hyperplastic primary vitre—ousなどの名称で報告されているものについて症例をあげて報告し，いささか論じてみることとする。

Twelve cases of posterior persistent hyper-plastic primary vitreous located at the optic nerve head or morning glory syndrome were reported.

The presenting symptom in those cases re-ported here was a unilateral squint with im-paired visual acuity, and no associated systemic diseases. Ophthalmoscopy revealed a white tis-sue at the nerve head, vascular anomalies, and peripapillary chorioretinal pigment disturbance. In two cases, retinal detachment occurred aro-und the disk.
