トラベクレクトミーの奏効機序

A clinical and histological survey of trabeculectomy

　原発開放隅角，原発閉塞隅角，先天，続発，嚢性緑内障計69眼を対象に，採取した組織片にトラベクルムを含んでいることを光顕下で確認し，術後1カ月から3年までの経過を観察した。

　手術方法はトラベクルムを含むたて長の強角膜片を切除，強膜弁の縫合を十分に行った。術後の眼圧コントロール良好例は95.7％であった。

　術後濾過瘢痕を形成したもの27.5％，このうち眼圧コントロール良好のもの94.6％であった。術後濾過瘢痕を形成しなかったもの，観察中に消失したものは72.5％で，このうち眼圧コントロール良好のもの96％であった。濾過瘢痕形成の有無による眼圧コントロールの差は認められなかった。

　トラベクレクトミーの奏効機序として結膜下への濾過のみでなく，脈絡膜外隙への房水流出路も考慮すべきであると考えた。

A clinical and histological survey was performed on 69 trabeculectomies performed for open-angle, angle-closure, congenital, secondary, and capsular glaucoma. It was discussed whether trabeculectomy is a bleb forming operation or not. The duration of follow-up ranged from 1 month to 3 years.

The surgical procedure used in all cases was the same, and the sclero-corneal specimen, 2-3 x 3 mm in size, was removed, and relatively tight scleral sutures were placed. The histological examination showed that all specimens contained trabecular meshwork and Schlemm's canal.
