Congenital external fistulae of the lacrimal sac.:Lacrimation and its treatment

　先天性外涙嚢瘻に起こる漏涙を止める確かな方法は瘻管の全切除である。フルオレスセイン残留試験と，瘻孔を涙点拡張針でふさいで行う洗浄試験で，鼻涙管に明らかな通過障害が認められなければ，痩管切除が流涙を招くことはない。

　瘻孔に通した通常の涙管ブジーを目標に，瘻管をその基部まで剥離する従来の方法の変法として，鳩日付きの瘻管同定ブジーを作り，これを瘻管に取り付けておくと，その全切除が容易である。本法を3例に試み，いずれも合併症なく，好結果を得ている。

An ideal treatment of lacrimation from con-genital external fistula of the lacrimal sac would be to tatally excise the lacrimal anlage duct. In the absence of blockage of the nasolacrimal duct, as proven by fluorescein disappearance test and irrigation test while occluding the fistula, the excision will not cause epiphora postoperatively.

I modified my previous technique of dissecting the fistulous tract up to its base using a lacrimal probe as a guide. A probe with an eyelet is used to locate the tract proved useful in facilitating the surgery.
