A 54-year-old female presented with swelling in the medial canthus and along the lower canaliculus in her left eye. She was tentatively diagnosed as diverticulum of the canaliculus. A giant yellow-white mass extending to the lacrimal sac was detected during surgery, leading to the diagnosis of canaliculosaccular fistula. The mass was identified as a becterial colony consisting of α -streptococcus species. Persistent infection appeared to have led to the formation of bacterial colony and fistularization involving the lacrimal sac.
