巨大な菌塊を呈した涙小管涙嚢瘻

A case of lacrimal canaliculosaccular fistula clogged by a giant bacterial colony

（P3-2-12）　巨大な菌塊を呈した涙小管涙嚢瘻の1例（54歳，女性）を経験した。難治性の左眼眼脂で当科を紹介された。左眼下眼瞼結膜内眼角部の発赤，腫脹，下涙小管に一致した皮膚の発赤，硬結があり，涙道通水検査では下涙点は通水不能，逆流，排膿がみられ，涙道造影，エコー検査で涙小管憩室を疑われ，摘出術を行った。術中，涙嚢に及ぶ巨大なmassが摘出され，ブジーにて涙小管から涙嚢への交通が確認された。本症例では，病理学的に確信はできなかったが，おそらく放線菌による涙小管炎から，菌塊を形成しながら涙小管瘻となり，涙嚢と交通したのではないかと考えられ，難治性結膜炎の症例では，放線菌による涙小管炎も念頭におくことが大切と思われた。

A 54-year-old female presented with swelling in the medial canthus and along the lower canaliculus in her left eye. She was tentatively diagnosed as diverticulum of the canaliculus. A giant yellow-white mass extending to the lacrimal sac was detected during surgery, leading to the diagnosis of canaliculosaccular fistula. The mass was identified as a becterial colony consisting of α -streptococcus species. Persistent infection appeared to have led to the formation of bacterial colony and fistularization involving the lacrimal sac.
