Limbal-based conjunctival flap is routinely used for filtration surgery. With this procedure, the con-junctiva tends to adhere to the sclera forming a scar tissue posterior to the filtrating area.

We started to employ fornix-based conjunctival flap because of several advantages as : easiness in dissecting and suturing, less liability of accidental damage of the flap as it is absent in the surgical field, and less interference with the subconjunctival aqueous flow.

We performed trabeculectomy with fornix-based conjunctival flap by limbal incision in 11 eyes in-cluding 5 cases of combined cataract extraction.
