Vitrectomy応用による完全脱臼水晶体摘出

Extraction of posteriorly luxated lens by means of vitrectomy

　硝子体中に完全脱臼した水晶体を硝子体手術を応用して摘出した9症例10眼について，その術式，臨床経過につき報告した。

　術式は（1） Open sky vitrectomy後に硝子体手術用クライオにて摘出する方法。

（2） Pars plana vitrectomy後にopen skyにて硝子体用クライオにて摘出する方法。

（3） Pars planaよりvitrectomy,lentectomyを行う方法。これらの3術式は安定した力法であるから，従来よりも積極的に手術を行うべきであると考える。すなわち，重篤な合併症が生じた後では視力回復が困難であるから，脱臼水晶体が成熟白内障となれば摘出術の適応と考えられる。

We performed surgical removal of completely dislocated lens in the vitreous cavity in 10 eyes of 9 cases with the use of one of the following techniques. 1) Dislocated lens was extracted transpupillary with cryoprobe after open sky vit-rectomy. 2) Dislocated lens was removed trans-pupillary with cryoprobe adjunct to pars plana vitrectomy. 3) Dislocated lens was removed by lentectomy through pars plana approach. The surgery was successful in all but one case of postoperative retinal detachment presumably due to prior phacolytic uveitis.
